The professionals at Sunrise Home Health Care work with patients and their loved ones to put together a skilled team of nurses, rehabilitation therapists, social workers, and certified nursing assistants according to each patient's needs.

Our services include physical therapy, iv therapy, speech therapy, occupational therapy, and medical social worker and trained medication administration services.

Our staff also educates patients about taking the best care of themselves, providing the tools they need to become self-sufficient and stay as healthy as possible in the communities where they feel the most at home.

Skilled Nursing Care

Sunrise Home Health Care's skilled staff of registered nurses performs a wide array of vital tasks for patients while regularly coordinating with their doctors and other health care providers. We pride ourselves in the professional and personal dedication of our nurses.

IV Therapy

We can teach you to administer your own injections and pre-measure medications to prevent dosage errors. Or, we can also administer injections for you, as frequently as needed. Sunrise will work with your doctor to assess treatment, dosage and provide comfort along with support.

Wound Care

Basic wound care includes cleaning wounds, changing wound dressings and administering antibiotic therapy (if prescribed). Along with self-care we train and educate patients and caregivers on how to change wound dressings, monitor healing, manage pain and recognize potential complications.

Medication Administration

This service is to help you organize your medication for an easy and effective system of tracking daily dosages. Our trained nurses will pre-pour or pre-measure medications for you and assess the effectiveness of medications and monitor potential side effects. We will also work with your doctor and pharmacist to prevent dangerous side effects.

Certified nursing assistant Care

An alternative to nursing-home care, the Managed Long Term Care (MLTC) plan is for adults who wish to live at home but need assistance with day-to-day activities. The program is designed to provide each member with the very best possible care. We encourage patients to take an active part in their own health care, and we offer many choices of services under physician orders.

Physical Therapy

If you or a loved one faces any number of conditions that make it challenging to move, your doctor may prescribe physical therapy. Occupational Therapy

Sometimes a stroke, illness, or injury can impact your fine motor skills and capacity to take care of yourself. Occupational therapy can help you or a loved one regain the capability to enjoy everyday activities, such as eating, dressing, and bathing, as well as preparing a meal or enjoying a favorite hobby. Our occupational therapists will develop a specific plan to meet your needs, and work with you to relearn necessary motor skills.

Speech Therapy

Speech therapy encompasses much more than relearning or refining your ability to form words, and it can service people of all ages. You or a loved one may have suffered a stroke or other illness that has affected the muscles that control swallowing and help you form words.
